Ans:
NHI IC cards are required for subsequent treatments following a doctor" + String.fromCharCode(39) + "s diagnosis. However, those treatments will not count as entries against the quota.

Ans:
After adding your baby as a dependent to your insurance, complete an "IC Card Application Form", attach a photocopy of your identification documents and mail to an NHI branch or liaison office. The application can also be processed in person at an NHI branch.
3. Is my personal information protected while using the IC card?
Ans:
NHI IC cards require a card-reader to access the stored information. Card-readers in various locations can only read your IC card once fitted with a specified security module only available from NHI branches. The mechanism is under strict control and requires cross-recognition so as to prevent unrecognized access. Patient" + String.fromCharCode(39) + "s medical histories are not written on the cards, so everybody need not worry.
4. How should I look after my IC card?
Ans:
NHI IC cards need to avoid high temperatures, dampness, magnets and metal items. They shall not be overly bent or the chips will be damaged and become inaccessible.

No. When applying for a new card, the previous one becomes automatically annulled. Such a card cannot be used again if recovered.
6. What to do if you don't have a NHI IC Card?

- If you have already joined the NHI program, but you have not received the NHI IC Card yet; or you are in the process of applying a new card due to name changes, card deteriorations, photo changes, lost, and etc., what will happen when you go to see a doctor? You just need to fill in the “Exceptional Medical Seeking Name List” at the clinic or the hospital, and you will still be treated as the NHI insured.
- If you have not received the NHI IC Card, or you have not you filled out the “NHI IC Card Application Form”, please fill one out as soon as possible (NHI branch offices, liaison offices, and post offices.) Attach your photo and a copy of your ID with the application form and mail or bring it in person to a nearby NHI branch office. The Bureau of National Health Insurance will issue your NHI IC Card as quickly as possible.
7. How about the specifications of the photo for the NHI IC Card? Can I use a photo without the requirements?
Ans:
This photo should be 2-inch, colored/black-and-white and taken within 2 months. On the photo, the figure shall be without a hat and shall not wear the colored glasses. We don't accept any photo other than a certificate photo. If the specifications of your photo are almost close to the above-mentioned but only the size is 1 inch, we could accept it. However, the clarity of the photo on the NHI IC Card will not be good as the 2-inch one.
8. What are the contents in NHI IC Card?
Ans:
There are four sections of information stored in NHI IC Card, including the personal information, the NHI-related information, the medical services and the public health administration. The contents are as follows:
- The Personal Information Section mainly includes the card's serial number, the cardholder's name, gender, date of birth, ID number, picture and the date of issue.
- NHI-related Information Section mainly includes the remark of the cardholder's status, the remarks for catastrophic diseases, the number of visits and admissions, the utilization of the NHI health prevention programs, the cardholder's premium records, the records for accumulated medical expenditures, the amounts of cost sharing, etc.
- Medical Services Section mainly includes the drug allergic history and the long-term prescriptions of the ambulatory care and certain medical treatments. This section will be gradually phased in depending on how the healthcare providers adapt themselves to the system.
- Public Health Administration Section mainly includes the personal immunization chart and the willingness for organ donation.
9. How do I renew the NHI IC Card If I have used it for six times already?
Ans:
You can update your card at the Public IC Card Kiosks installed at all BNHI branch offices and affiliated outpatient centers; or look for the IC Card Readers installed at nearest hospitals, or administration offices.
10. What shall I do if I lost or damaged my card; or if I need to change my name or photo?
Ans:
You can go to each BNHI service center or post office to fill up “the NHI IC Card application form”, and paste your identification card copy on the back, then give it to service windows to apply a new card. There will be a NT$200 handling charge, unless the reason for applying a new card is not your responsibility.
