In response to the challenges posed by Taiwan's aging population and the prevalence of multiple chronic diseases, the National Health Insurance Administration (NHIA) launched the Family Physician Program 2.0 and the Holistic and Community Healthcare Plan in 2024 to provide more comprehensive care for chronic diseases.
Family Physician Plan 2.0:
Building upon the existing family physician program, this healthcare model fully integrates chronic disease care through health management by family physicians. It consolidates multiple outpatient services for chronic disease management, facilitates patient referrals and follow-ups, and enhances the overall quality of life for individuals with chronic conditions.
District Hospital-Led Community-Based Holistic Healthcare Model:
District hospitals establish care teams to focus on patients with the "Three Highs"—high blood pressure, high cholesterol, and high blood sugar. This model provides integrated services to reduce the risk of severe chronic diseases and aims to achieve the 888 Policy Goals for the prevention and treatment of the "Three Highs."
The Family Physician Platform:
The Family Physician Platform integrates health management resources, consolidating information on the "Three Highs," preventive healthcare, cancer screenings, and lifestyle habits. It enables physicians to closely monitor patients' health while helping the public better understand their own health conditions. The platform's beta version was launched on December 6, 2024, with Phase 1 prioritizing over 5,000 clinics participating in the Family Physician Plan. The system is expected to be gradually expanded to various levels of medical institutions.
