Under this program, community healthcare institution work together to provide comprehensive healthcare to patients with diabetes, freeing them from the hassle of traveling to different locations. This program ensures that patients receive time-saving and reliable healthcare services.
Community healthcare institutions, formed by local clinics and hospitals in nearby neighborhoods, collaborate to provide personalized and continuous care for patients with diabetes. Professional medical teams, including doctors, nutritionists, and case managers, are assembled to offer comprehensive care. This care includes health management, a 24-hour medical consultation hotline, referrals, preventive care, disease management, risk factor prevention, lifestyle intervention, and ongoing healthcare for patients with chronic conditions. Additionally, with the support of the NHI App and My Health Bank, digital care tools enable patients to access their blood sugar test results, medication profiles, and follow-ups within the Diabetes Program.
The NHIA has established an effective referral and collaboration mechanism for medical institutions within a community through the "Family Physician Program." The aim of this program is to enable the NHIA and family physicians to safeguard public health.
