In order to help the aging population and patients with post-acute care (PAC) disability, and to lessen the burden of the healthcare system, families, and society, the National Health Insurance Administration (NHIA) has launched the Post-Acute Care (PAC) Program since 2014. Further, since July 1 2015, in addition to cerebrovascular accidents and burns and scalds, the program has extended coverage of traumatic nerve injuries, fragility fractures, heart failures, and fragile elderly patients. Moreover, other than the models of inpatient care and day care, the home health care was also introduced.

Currently, there are five inter-hospital PAC teams in Yunlin, Chiayi, and Tainan, covered by the Southern Division of the NHIA. There is a total of thirty-three participating hospitals (15.9% of all hospitals in the country). Among them, seven hospitals provide day care while thirteen hospitals provide home care. To sum up, 6,227 patients (16.2% of all patients in the country) of aforementioned six categories of cases have been accepted into the PAC Program in the past five years. Out of all the closed cases, 83.6% of the patients showed significant improvement in terms of abilities and 86.6% were able to return home. The overall result shows that 92.9% of the patients has been steadily and rapidly improved.
