The Care Integration Project for Diabetes and Early Chronic Kidney Disease was adopted on December 9, 2021. This project addresses comorbid patients with diabetes and early chronic kidney disease because two diseases have multiple concurrent primary conditions and a causal relationship. Therefore, hospitals are encouraged to establish integrated care teams, and use resources effectively. Health education and follow-ups are also highly encouraged in order to help patients take part in disease management.
