Replacing and reissuing the NHI card

I. On-site Application

      (I) Reasons for processing: Loss, damage, replacement of photo, change to personal information (those changing the national ID number will not receive the card on the same day. Please ask or mail the counter to request information on when the card will be available).

      (II) Documents

  1. One 2-inch, upper body, front-facing, photo with no hat, no color-lens glasses, with clear facial features taken recently. In consideration of clarity of identity, the photo may not be edited or superimposed.
  2. Original identity document of the intended cardholder (and representative).
  3. Processing fee of NT$200.

      (III) Locations

  1. NHIA regional division or liaison office.
  2. Local administrative office authorized by the NHIA.

      (IV) How to pick up the NHI card

Receive the card on site (those changing the ID number will not receive the card on the same day. Please ask or mail the counter to request information on when the card will be available).

      (V) Payment methods

  1. Cash.
  2. Credit card (check with the issuing bank for any handling fees).
  3. Electronic payment (some counters of the NHIA accept payment by EasyCard or iPASS. Check for payment options with the counter at time of payment.).

      (VI) Required time

Depends on the situation on the day, but usually takes 15 to 30 minutes.

II. Post Office Collection

      (I) Reasons for processing: Replacements of Lost or Damaged Cards or Cards with Name or Photo Changes

      (II) Documents

  1. One 2-inch, upper body, front-facing, photo with no hat, no color-lens glasses, with clear facial features taken recently. In consideration of clarity of identity, the photo may not be edited or superimposed.
  2. Original identity document of the intended cardholder.
  3. One photocopy of identification document (front and back)
  4. NT$200.

      (III) Locations for processing: All post offices.

      (IV) How to pick up the card: Sent to address provided on the NHI Card Application Form by registered mail.

      (V) Payment method: Cash.

      (VI) Required time: 5 to 7 working days.

III. Household Registration Office

      (I) Reasons for processing: Changing personal information, or for when ID card and NHI card are lost at the same time.

      (II) Documents: Inquire of the Household Registration Office for the documents and photos required.

      (III) Locations for processing: Household Registration Offices

      (IV) How to pick up the card: Sent to address provided on the Application Form by registered mail

      (V) Payment methods: Take the payment slip to pay at an ATM, convenience store, or financial institution. Different handling fees may be charged by each payment method.

      (VI) Required time: 4 to 6 working days.

IV. Township (Town/City/District) Office

      (I) Reasons for processing: Loss, damage, replacement of photo

      (II) Documents

  1. One 2-inch, upper body, front-facing, photo with no hat, no color-lens glasses, with clear facial features taken recently. In consideration of clarity of identity, the photo may not be edited or superimposed.
  2. Original identity document of the intended cardholder (or representative)
  3. NT$200

      (III) Locations for processing: For those insured at their township/city/district office, apply at the respective administrative office.

      (IV) How to pick up the NHI card: Sent to address provided on the Application Form by registered mail.

      (V) Payment methods: Take the payment slip to pay at an ATM, convenience store, financial institution, or e-Bill. Different handling fees may be charged by each payment method.

      (VI) Required time: 2 to 5 working days

V. Online application

      (I) Reasons for processing: Loss, damage, replacement of photo

      (II) Application methods:

1.Personal online application
  (1) Online platform (a digital certificate required)

The applicant uses a Citizen Digital Certificate (or registered NHI card) to log in to the Personal NHI Information Network Services (they can also apply for their dependents).

            (2) Online platform (no certificate required)

Please go to NHI card service online platform (no-certificate-required area, speedy pick-ups). The applicant needs to verify their household number, enter information, pay fees, and set the card collection time and service location. They can then quickly collect the NHI card at the designated location and designated time (they can also apply for their dependents).

            (3) NHI APP’s E-Counter

The applicant downloads the NHI APP on a mobile phone, and logs in to the "E-Counter" for processing (they can also apply for their dependents).

2. Insured unit

(1) Online platform (a digital certificate required): The insured unit uses a Citizen Digital Certificate authorized by this unit certificate (or the registered NHI card of the person in charge) to log in to multi-certificate online platform for processing (the insured unit can apply for the insured or their dependents in that unit)

      (III)Payment method and required time for applying for replacing or reissuing the NHI card online:

VI. Free replacement of NHI card

      (I) Conditions for free replacement of NHI card

  1. Bending, scratches, deformation, chip loss, discoloration, or wear that are not human-caused and lead to inability to use the card.
  2. Foreigners who obtain a new UI No. from January 1, 2022, the NHIA will directly collect the necessary information and archived photo files from the National Immigration Agency, and then make and send the NHI card with new UI No. free of charge.

      (II) Where to apply for

      1. On-site Application in Person at NHIA

                 (1) Documents

                      i. One 2-inch, upper body, front-facing, photo with no hat, no color-lens glasses, with clear facial features taken recently. In consideration of clarity of identity, the photo may not be edited or superimposed.
                     ii. Original identity document of the intended cardholder (or representative).
                     iii. Old NHI card.
                 (2) Locations

 The NHIA’s regional divisions and its liaison offices.

                 (3) Depends on the situation on the day, but usually takes 15 to 30 minutes to pick up on premises

       2. Submitted via mail

                (1) Fill out the Application For NHI Card Form and return it together with the old card to a regional division of the NHIA.)

                (2) The new card will be mailed to the designated address within 5 to 7 working days.

       3. Foreigners who obtain a new UI No., the NHIA will directly make and send the new NHI card with new UI No. to the applicants. Before receiving the new NHI card, the old card can still be used for medical treatment. If there is an urgent need to renew your NHI card, you can apply at a NHIA regional division.
